The In-Home Family specialist will work in partnership with and be provided supervision by the Community Services Director, while independently providing advanced support to families, ren and youth who are experiencing a variety of stressors in order to increase family functioning, relationships and needed resources while reducing the likelihood that ren and parents will become separated, isolated, disempowered or disengaged.
Representative Tasks/Responsibilities
- Develop relationships with family members, acting as a mentor
- Provide comprehensive family support services to include:
- Advocacy
- Case Management
- Parent Education and Training – “Teaching Family Model”
- Serve as an effective liaison, as necessary
- Develop a Family Success Plan or Treatment Plan in collaboration with therapist identifying short-term and long-term goals (or assist parents in meeting goals identified by OCS)
- Provide weekly in-home support or as needed
- Maintain documentation, write monthly report
- Attend and participate in individual and group supervision according to JYS policies
- Perform other related duties as assigned.
- Ensure that the privacy and confidentiality of client, family and health information is maintained
- Offer discharge and after care planning

Supervises: None
Knowledge and Ability
- Understanding of case management to include community resources and referral processes
- Ability to acquire (through JYS training), provide and model parent skill development from the Teaching Family Model
- Understanding of trauma responsive care
- Ability to maintain a calm demeanor and diffuse tense and stressful situations
- Comfort level to provide mentorship to families in the home environment
- Ability to communicate effectively, both orally and written, document family visits and write reports
- Efficiently and effectively organize and prioritize work
- Contribute positively as a team member and work effectively with other internal and external stake holders
- Knowledge of the indicators of abuse and neglect and mandatory reporting laws
- Knowledge of the principles of development, behavior management, and crisis intervention
- Knowledge of cultural considerations in the treatment process
